# What repeated blows to the head in sport may mean for dementia risk
Recent research has identified a correlation between repeated head impacts in contact sports and markers of poorer brain health in former athletes, though scientists stop short of proving a direct causal link to dementia.
Studies examining former football, rugby, and ice hockey players show measurable changes in brain structure and function associated with accumulated head trauma. These include alterations in white matter, the tissue that connects different brain regions, and reduced cognitive performance on standardized tests. Some research also documents increased tau protein and amyloid beta, hallmarks of Alzheimer's disease, in former players with histories of multiple concussions.
The evidence presents a clear concern for schools and youth sports organizations. The cumulative effect of subconcussive impacts—smaller collisions that don't produce diagnosed concussions—appears to damage brain tissue over time. Young athletes face particular vulnerability because their brains are still developing.
What remains uncertain is whether these brain changes inevitably lead to dementia or cognitive decline later in life. While the biological pathways connecting head trauma to neurodegenerative disease appear plausible, long-term follow-up studies tracking former athletes into their 70s and 80s remain limited. Some individuals with significant head impact histories show no cognitive problems, suggesting other factors like genetics, overall health, and education level may modify risk.
Educators and athletic directors confront a practical challenge. They cannot eliminate heading in soccer or contact in football without fundamentally changing these sports. Instead, prevention strategies focus on reducing unnecessary impacts through proper tackling technique, enforcing strict concussion protocols, and limiting contact drills in practice.
The Centers for Disease Control and the American Academy of Pediatrics recommend age-appropriate restrictions on full-contact practices for young athletes. Some youth leagues now prohibit heading before age 11 in soccer.
